Patient-Centered Medical Home Certification

Shasta Community Health Center has achieved Patient-Centered Medical Home Certification and Dental Home Accreditation by the Accreditation Association for Ambulatory Health Care (AAAHC). Healthcare organizations that earn AAAHC Certification demonstrate dedication to high-quality patient health care and become part of an elite force of organizations that prioritize quality for their patients and staff. Status as an organization that has earned certification means Shasta Community Health Center has met the highest level of nationally recognized standards for the provision of quality health care set by AAAHC. Certified Medical Respite Care Program

We are pleased to share that our medical respite care program, Shasta Community HOPE Medical Respite, has become certified by The National Institute for Medical Respite Care, a special program of the National Health Care for the Homeless Council! This means that we have completed a rigorous process to demonstrate a high degree of fidelity to nationally recognized principles, standards, and expectations in the field of medical respite care. Shasta Health Rock Stars

The Shasta Health Assessment and Redesign Collaborative (SHARC) is a dynamic group of health care leaders that has been working to improve access to quality affordable healthcare in the North State since 2009. The collaborative is facilitated by the Health Alliance of Northern California (HANC). SHARC’s new Shasta Health Rock Stars program seeks to increase visibility of health career opportunities for area youth and recognize local health care professionals who have devoted their careers to service of Shasta County’s underserved populations. 

We are honored that several of our staff at SCHC have been recognized as Shasta Health Rock Stars.

ADCES DEAP Diabetes Education Accreditation Program

Diabetes Self-Management Education and Support (DSMES)

Shasta Community Health Center’s Diabetes Self-Management Education and Support (DSMES) program is accredited by the Association of Diabetes Care & Education Specialists (ADCES) through its Diabetes Education Accreditation Program (DEAP), a national accrediting organization recognized by the Centers for Medicare & Medicaid Services (CMS).

Accreditation means our DSMES program meets the 2022 National Standards for Diabetes Self-Management Education and Support and provides evidence-based, person-centered diabetes education and support. The program is designed to help individuals build the knowledge, skills, and confidence needed to manage diabetes in daily life.

Accreditation applies specifically to the DSMES program and does not extend to other services provided by Shasta Community Health Center.

Federally Qualified Health Center (FQHC)

We are a FQHC and receive funding and support through the Health Resources and Services Administration (HRSA), which is a branch of the Department of Health and Human Services (DHHS). FQHCs are nonprofit health centers or clinics that are governed by a community board that is at least 51% made up of health center patients. FQHCs provide primary and preventative care for medically underserved communities on a sliding fee scale based on ability to pay.

Federal Tort Claims Act (FTCA)

Shasta Community Health Center (SCHC) is a state licensed and independent 501(c)3 non-profit Community Health Center, led by an independent and consumer majority board of directors. SCHC is also a Federally Qualified Health Center (FQHC). As such, SCHC is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). SCHC receives Health and Human Services funding and has Federal Public Health Service (PHS) status with respect to certain health or health-related claims, including professional liability claims (i.e. malpractice claims), for itself and for those covered clinicians/practitioners, administrative and support staff along with Board members of SCHC.
