THE HOPE PROGRAM: Health Outreach for People Everywhere
The mission of the HOPE Program is to deliver primary health services, essential for human wellness, to the most vulnerable citizens of our community.
Our HOPE Program is a continuum of care for our unsheltered community. The program operates through our system of community health centers in Redding, Anderson, and Shasta Lake which provide medical, dental, vision, mental health, and substance use disorder services, as well as case management and referrals.
The HOPE Team cares for patients in multiple settings:
- Street Outreach - Members of our HOPE Team provide "street medicine" to unsheltered people experiencing homelessness in locations like encampments, parks, and other places in our community.
- Mobile Medical Van - The HOPE Team operates a 40-foot mobile medical van that is equipped with a lab area, intake waiting area, and two exam rooms. This van operates in established community settings three days a week.
- Shelter Clinic - The HOPE Team sees patients at the Good News Rescue Mission two days per week.
- Medical Respite - Members of HOPE Team provide acute and post-acute care for persons experiencing homelessness who are too ill or frail to recover from a physical illness or injury. Medical care and case management are provided at the Hartman House and other local hotels.
Certified Medical Respite Care Program
We are pleased to share that our medical respite care program, Shasta Community HOPE Medical Respite, has become certified by The National Institute for Medical Respite Care, a special program of the National Health Care for the Homeless Council! This means that we have completed a rigorous process to demonstrate a high degree of fidelity to nationally recognized principles, standards, and expectations in the field of medical respite care. Donations
The demand for care in the HOPE program is at an all time high. We greatly appreciate any donations to help fund this much needed program. Donations help us provide items such as food, toiletries, hygiene products, undergarments (socks, underwear, etc.), blankets and sleeping bags, backpacks, and clothing for job interviews.
